Services NOT provided

Forensic psychiatry Evaluations- (e.g., Custody evaluations, Guardianship evaluations, Disability evaluations, Legal Testimonies, etc.)
Consumers are required to employ independent forensic psychiatric services if this type of evaluation or testimony is needed.

Geriatric psychiatry
(patients 65 years of age and older will not be accepted as New Patients)

Substance abuse treatment
Services are not provided for Substance use D/Os as one’s primary psychiatric diagnosis or comorbid substance use disorders not in remission.

Pain management
Services are not provided for Substance use D/Os as one’s primary psychiatric diagnosis or comorbid substance use disorders not in remission.

Driver License Eligibility
Psychiatric evaluations not performed and no forms going to be filled out for Diver license eligibility including Commercial Driver Licenses (CDL). Individuals will be directed to designated service agencies that provide such services. Your treatment record can be provided to a concerned third party with your written consent.

Moderate to Severe Intellectual Disabilities
Intellectual disabilities need multi-specialist team approach. Moderate to Severe Intellectual Disabilities need more intensive specialized treatments in addition to Medication Management, that our clinic cannot provide.

Evaluations

Telepsychiatry: Initial Psychiatric Evaluation $350 / Follow Up $125 (15 min), $200 (30min)

Telephone Calls with Providers: Free for less than 5 minutes and then $30 per each 5 minutes increments.

Medical Records: Copying of medical records will incur a minimum of $25 fee. If paperwork is more than 20 pages, additional charges will apply (0.50 cents for each additional page along with the cost of delivery of records) Medical Records will be forwarded or faxed to your provider free of charge for continuation of care with your consent.
